Club Registration form

If you are interested in creating a new student club, please fill out the above Club Registration Form. After completion of the form, you will be contacted by the Student Life Committee with next steps.听

Club/organization Roster form

听

Please submit a Club Roster form with student names and student ID's to the Student Life Committee upon completion of the club registration form. This ensures the minimum membership is met to start a club. Club's will be asked to submit an updated Roster form every semester.

Student Conduct hazing form

听

黑料吃瓜网 prohibits any form of hazing on or off campus. Please fill out the Student Conduct Hazing form above if you have witnessed a hazing event.听
